Case management serves as a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. As a Telephonic Case Manager you will be responsible for assessing, planning, implementing, coordinating, and monitoring health care services to meet an individual's health needs through communication and available resources to promote quality, cost-effective outcomes. You will work collaboratively with the member and their provider(s), to assist patients and their families access needed services, optimize effective self-care strategies, identify options and make choices that reflect their needs and priorities.Key Responsibilities:o ASSESSMENT - Conduct initial and follow-up assessments within designated timeframes on members identified as having complex case management needs. Collect in-depth information regarding a patients clinical history, including co-morbid conditions, prognosis, treatment plan, response to treatment, access to care, access to personal and community resources, utilization of care, personal coping mechanisms, learning needs and financial constraints. This information provides the foundation for the identification of patient problems and the development of a comprehensive care plan that will address identified needs. Identify any gaps or barriers that would impact compliance with the prescribed treatment plan.o PLANNING- Develop specific short and long-term goals, objectives, and interventions with accompanying timeframes. Engage member, family, caregivers, and healthcare providers to assure that a well-coordinated treatment plan is established. Utilize holistic approaches to member care that integrates member's life and motivational goals into the treatment plan. Prioritize care needs, set goals and develop a treatment plan (or plan of care) that also addresses gaps and/or barriers to care and uses evidence-based practice as the foundation. o IMPLEMENTATION- Identify and carry out the relevant interventions that will facilitate the accomplishment of goals established in the care plan. Track the member's health status and progress in achieving clinical and personal goals. Provide education, information, direction, and support related to care goals of patients. o COORDINATION- Identify, organize, and integrate the appropriate acute, rehabilitative, sub-acute, homecare, alternative, and community resources needed to accomplish the identified goals in the care plan. Communicate with member's families, caregivers, physicians, and other service providers to coordinate the care needs for the member. Work to facilitate member compliance and to ensure continuity of care. Facilitate problem resolution with members, providers, and other agencies or entities as needed. o MONITORING - Collect sufficient data from all relevant resources to determine the effectiveness of the care plan. Track the member's health status and progress in achieving clinical and personal goals. o EVALUATION- Make repeated assessments of the care plans effectiveness at appropriate intervals and make plan modifications, as necessary, to achieve patient goals and improve outcomes. Monitor and evaluate the member's response to treatment(s). Education/Skills/Experience:o At least two years full-time equivalent of direct clinical care to the consumer.o Active-RN License in good standing in the practicing state o BSN preferredo 3 years surgical/medical nursing experience.o Recent experience as a complex case manager.o Home health experience a plus.o Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, and time management skills. o Must demonstrate the ability to work effectively in an interdisciplinary team format.o Proficient Microsoft computer skills. o CCM (Certified Case Manager) preferred.
