KEY REQUIREMENTS

U.S. Citizenship
Full unrestricted Licensure, Certification, or Registration required.
Must pass pre-employment examination.

English Language Proficiency required. DUTIES:
This RN will work with the Care Coordination/Home Telehealth Program (CCHT) team, utilizing the latest in communication technology to communicate with patients in their homes to facilitate care. Â"TelebuddyÂ" communication equipment is installed in veteranÂ's homes for rapid, efficient transmission of health data (such as changes in weight, symptoms, etc.) for patients with conditions necessitating close monitoring. The CCHT team assesses needs based on incoming data, and calls patients to further assess need for intervention, such as prescription change (by provider), primary care office visit, or emergency room visit.

  • BSN preferred;
  • Three years of successful clinical experience required;
  • One year geriatrics experience preferred;
  • Certified Diabetic Educator (CDE) preferred.
Provides initial and ongoing assessment of patients to identify needs issues, care g! oals, and appropriate resources necessary for care management;

Functions as a systems coordinator for the plan of care; monitors progress through the continuum of care and intervenes as appropriate to facilitate achieving patient outcomes within anticipated timeframes;

Collaborates with patient and care providers in any and all settings where care is being provided to evaluate and update changes in the therapeutic plan of care and patient management;

Recognizes complex situations that impact patient care and intervenes, using sound judgment, professional attitude, and appropriate channels;

Recognizes impact of age-specific care needs;

Maintains a working knowledge of resources available in the community;

Participates in performance improvement activities related to the service line quality improvement process;

Develops and implements interdisciplinary standards of practice and care at the station level for the C! are Coordinator role;

Participates in VISN-wide standards deve! lopment for the role;

Demonstrates equipment competencies in setting up, using, and troubleshooting
equipment used in the telehealth project;

Provides training for others;

Establishes ongoing relationships with professional/health related groups within the community;

Presents accurate information about the Telehealth project in interdisciplinary and public forms, without violating patient confidentiality;

Bases practice on current knowledge/trends/technological advances and/or research findings;

Participates or conducts interdisciplinary research-related activities as appropriate;

Advocates fiscal responsibility in the management of patient care through effective utilization of resources.

Relocation expenses will not be reimbursed.

QUALIFICATIONS REQUIRED:
Must be a citizen of the United States.

Must be a graduate of a school of professional nursing approved by the appro! priate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).

Must have a current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.

Must be proficient in spoken and written English.

Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience as a RN will be e! valuated to determine if the experience is of an acceptable level of qu! ality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research. Education/Experience Requirements:

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nur! sing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
