We are looking for new Team MembersRN's, LPN's and CNA's and Certified CaregiversThe Registered Nurse's and Licensed Practical Nurses's will assess, plan, implement, evaluate and care fo! r assigned patients in Home Health Situations. Certified Nurses Assistants and Certified Care GiversWill work in a Home Health Situation, helping patients and clients with Activities of Daily Living. We service the entire Phoenix Valley including Casa Grande.We currently have weekend positions available in the East ValleyJob DescriptionEnsure patients receive care and treatment prescribed by physicians.Provides direct physical, clinical, emotional and psychological care and support to patients.Observe, monitor, assess, report and document the conditions of patients and monitor their response to treatment.Administer medication in accordance with knowledge of pharmacological effects, benefits and side effects.Document times, dosage and other information according to policy and procedures.Deliver individualized care according to age appropriate standards of care, physician diagnosis and plan of care.Complete accurate and concise documentation on all assigned patient care recor! ds.Maintain with respect to patient privacy rights, confidenti! ality and HIPAA directives.Notify appropriate nursing and medical staff to changes in patient status.Foster constructive relationships as a member of patient care team by providing direction and assistance to other facility team members as needed.Follow policies and procedures related to assigned patient care and facility guidelines.May direct the patient care of others on the patient care team as assigned.
Requirements Current unrestricted Nursing License or Certified Care giving Certificate presented in original format for the state of employment and assignment. Minimum of 1 year of supervised clinical experience in the professional area and specialty in which you will be working. Completed skill competency checklist. • Successfully scored above 80% on validated clinical proficiency test as appropriate for each area of clinical specialty or online version at www.NurseTesting.com. Current, annual BCLS, ACLS or appropriate cardiac life support certification for specia! lty. Drug screen and criminal background check to be performed prior to hire and assignment. Current statement of satisfactory health, including Hepatitis B documentation, negative Tuberculin test results or chest x-ray as required. Satisfactorily completed I-9 requirements.
