JOIN US NOW as we are seeking a CLINICAL SOCIAL WORKER who is passionate about the health and well-bein! g of our patients to be responsible for clinical social work services to the culturally diverse and all ages population supporting the hospital's mission and philosophy and to perform clinical assessment and reassessment of patients and their family needs, provide crisis intervention and brief counseling with special attention to the psychosocial and psychiatric effect of illness and disability, including issues of death and dying, participate in multidisciplinary patient care planning, and ensures continuity of care by identifying long term needs and coordinating referrals to appropriate hospital and community resources and provide consultation, education and psychosocial support to other health care professionals
MINIMUM REQUIREMENTS:
Master of Social Work degree
The clinical SOCIAL WORKERS at Chandler Regional Medical Center are integral members of the healthcare team by providing services to patients of all ages with significant psychosocial! , psychiatric, social, emotional, financial, educational and/o! r cultural needs. Utilizing intervention strategies designed to increase effectiveness of coping, strengthen informal support systems, and facilitate continuity of care, our clinical social workers are available on-site 17 hours/day, 7 days/week.
