More complete informations about this career opportunity please read the description below. We are in search of a Hospice Relations Coordinator for our Phoenix, Arizona market. This individual will assist in coordinating the hospice initiative tha! t engages local hospice organizations through programs and events that are intended to build relationships raise the status of our locations in the minds of the hospice community and increase awareness of pre-need education. In addition, this individual will assist in administrative tasks and with the follow-through of the monitoring and tracking.

SPECIFIC RESPONSIBILITIES
Builds relationships with professional caregivers in the hospice setting with a clear focus on execution and measurable outcomes
Rolls out, trains and monitors progress of our markets for the Hospice Relations program with a strong focus on accountability and hospice tracking in our COAR and HMIS systems
Develops local programs that will focus on educating hospice about the funeral and grieving process and the benefits of making arrangements in advance
Assists in administration, training and furthering of programs that focus on educating hospice about the funeral process, pre-need! and at need offerings and our service to hospice staff
Co! mpiles and organize hospice data for market management and present findings on conference calls and market meetings
Drives relationship building opportunities with hospice as necessary while holding self and others accountable
Assists in facilitating local market presentations and representing the company at conferences
Supports the hospice initiative and locations by scheduling appointments and CEU classes, coordinating logistics, handling calls and fulfilling requests
Coordinates multiple diverse projects with several variables, set realistic deadlines, and manage timeline

Job Requirements

Education:
LVN or higher in nursing or a Bachelor’s degree

Experience:
5+ years’ experience in a hospice or hospital setting pref! erred

C ertification:
Current Valid State issued Nursing License (preferred)

Knowledge, Skills & Abilities:
Ability to work independently and as an effective team member
Proficiency in Word, Excel, PowerPoint
