Position Summary:
The Nurse Case Manager utilizes their clinical skills telephonically.

Work in a positive office setting while still making a difference in member's lives. The Case Manager will collaborate and work closely with other healthcare professionals, including providers within the community and clinical team members internally. This position offers a supportive environment with professional development opportunities.

Additional Job Responsibilities:
The Nurse Case Manager is responsible for telephonically assessing, planning, implementing and coordinating all case management activities with members from our Federal Plans
The CM is responsible to evaluate the medical needs of the member in order to facilitate and promote the members overall wellness
The CM develops a proactive course of action to address issues presented to enhance the member's short and long term outcomes

Background/Experience:
Minimum of 3 years clinical experience required
Case Management experience preferred

Education:
The minimum level of education required for candidates in this position is a Associate's degree or equivalent experience

Licenses and Certifications:
Registered Nurse (RN) with active state license in good standing required

Functional Experiences:
Functional - Clinical / Medical/Direct patient care (hospital, private practice)/1+ Years

Technology Experiences:
Technical - Desktop Tools/Microsoft Outlook/1+ Years/End User
Technical - Desktop Tools/Microsoft Word/1+ Years/End User
