We are owned and operated by a former nurse. Currently, we are looking for an Admissions Registered Nurse in the East and West Phoenix Metropolitan Areas to provide exceptional care to our patients, families, group homes/facilities/hospitals, physicians, and prospective clients and patients. The Admission Nurse is responsible for the timely admission of all patients to the program, explaining Serenity Hospice philosophy and services, and ensuring quality communications between the location of the patient, physician, family members, and hospice team.
The nurse is also responsible to stay in compliance with all State, Medicare, and Serenity Hospice regulations.This position is for RN's only Experience: One Year previous nursing experience preferred.One year previous hospice nursing experience preferred. Education:Associate degree from an accredited Nursing School. BSN/MSN preferred. Licensed/Certification:Registered Nursing License from the Arizona Board of Nursing Required.

Manages admission process by ensuring proper communication with admit coordinator, physician, family, patients home, and clinical team.2. Conduct admission process in a timely manner and with outstanding professionalism.3. Contacts appropriate team manager for diagnosis, medication, and level of care4.
Completes documentation within 24hours and ensures accurate and complete documentation.5. Maintains and develops professional nursing practice for self and others. Pursues continued training in changes in treatments and techniques through obtaining CEU's. Is available as a resource for coâ€"workers.
Attends > 50% of staff meetings and all mandatory inâ€"services. Maintains skill set specific to needs of the patient population.6. Document care provided or other information in keeping with department policies7. Communicate effectively and tactfully.
Send out nightly report on admissions.8. Practice careful, efficient, and noâ€"wasteful use of supplies.9. Documentation must be turned in minimum once per week.10. Follows policies with regard to tardiness, attendance, dress code and use of time.
