One of out national long term acute care (LTAC) hospital providing focused medical, nursing and rehabilitation services for patients who need more acute medical care for their recovery process. DUTIES INCLUDE BUT ARE NOT LIMITED TO:
Provide patient care activities to ensure safe and effective drug therapy Be of service to patients, physicians, nurses and other pharmacist Maintain and improve job knowledge, skills and competence Articulate daily activities that support continuous quality improvement of patient care Demonstrate knowledge and mastery of required job functions, internal communications and operating systems Accurately enter orders in the computer in a timely manner Support and assist in meeting departmental goals for order entry and clinical programs Ensure facility procedures are followed when non-formulary drugs, target drugs, restricted drugs and investigational drugs are ordered Initiate change in assignments based on workload and resources available Demonstrate proficiency in computer order entry and related computer functions Screen for drug interactions, allergies, order duplications, appropriate diagnosis, renal and liver function prior to order entry Assure facility queues are addressed in a timely manner Investigate and report adverse drug events and medication incidents Facilitate dissemination of drug information in a timely manner to achieve high quality, cost effective, positive therapeutic outcomes when consulted Review and interpret all physician orders received, using the patient profile Monitor for incompatibilities, concentration and rate on intravenous drugs Participate in the maintenance of continuous quality improvement program Assist in the development and review of COE protocols Document clinical interventions and follow-up when indicated Assess orders for age-specific appropriateness from neonatal through geriatric Perform other duties as deemed necessary by the Pharmacy Manager or Division Director of Pharmacy, in accordance with established facility hospital policies and requirements of accrediting bodies

Job Requirements
- Must possess, as a minimum, a B.S. degree in Pharmacy from a NABP accredited College of Pharmacy program.
- Current State Pharmacist licensure required.
- Previous Acute Care Hospital experience required.
- Must be knowledgeable of Institutional Pharmacy regulations and practices.
- Experience with Pharmacy Data Entry systems as well as strong communication and interpersonal skills essential.
- Knowledge of intravenous solution programs an advantage.
- Able to work assigned/needed hours and accepts Operation’s “Customer Service†and “Staff Respect†philosophies.
