The RN-Nurse Consultant provides consumer specific support , discharge planning, and support to DDD consumers in Group Homes, Skilled Nursing Facilities, consumer homes, etc. Requires driving on a regular basis throughout Maricopa County to conduct home vists, nursing facility vists and to attend meetings for esstablished case load. Skills / Requirements
- Graduate of an approved school of professional nursing and currently licensed in the state of Arizona - license must be in good standing.
- At least two (! 2) years nursing experience, experience with individuals with developmental disabilities preferred.
- Must be able to complete pre-employment drug-screen and physical and be capbable of obtaining and maintaining AZ DPS FingerPrint Clearance. Important Notes
