Description: Hospice Family Care, Inc. is at present looking to employ Case Manager Registered Nurse Inpatient Unit right now, this position will be placed in Arizona. Detailed specification about this position opportunity please read the description below. The Case Manager Registered Nurse (RN) will provide direct patient care services to hospice patients, supportive care to the patientâ€! ™s family, manages an interdisciplinary group consisting of Licensed Practical Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care and case manages all assigned patient clinical records.
Required Skills
Currently a licensed Registered Nurse inArizonaor compact state.
CPR Certified, if applicable.
Case Manager RNs must have their own reliable transportation, a valid driver’s license, and proof of automobile insurance.
Understanding of computer usage is required; understanding of Microsoft applications is a plus.
Required Experience
A minimum of one (1) year experience, either in oncology, long-term care or home health. School internships and clinical hours may be considered as experience in individual cases.
