POSITION DUTIES:
Nursing Process

1.1 Assessment - An RN uses a systematic, dynamic way to collect and analyze data about a client, the first step in delivering nursing care. Assessment includes not only physiological data, but also psychological, sociocultural, spiritual, economic, and life-style factors as well.

1.2 Problem Identification - Poblem identification is the nurse’s clinical judgment about the client’s response to actual or! potential health conditions or needs.

1.3 Planning a! nd Outcomes â€" Based on the assessment and problem identification, the nurse sets measurable and achievable short- and long-range goals in collaboration with the patient and family. Assessment data, problem identification, and goals are written in the patient’s care plan so that nurses as well as other health professionals caring for the patient have access to it.

o Establishing daily goals with the patient and family

1.4 Implementation - Nursing care is implemented according to the care plan, so continuity of care for the patient during hospitalization and in preparation for discharge is assured. Care is documented in the patient’s record.

1.5 Evaluation - Both the patient’s status and the effectiveness of the nursing care must be continuously evaluated, and the care plan modified as needed.

1.6 Care Coordination - Actions initiated by nurses with patients, families, and/or members of their healthcare team to manage and correct the sequence, timing, and/or effectiveness of patient care from admission to discharge.

Prac! tice is consistent with the American Nurses Association “Code of Ethics for Nurses”. Apply knowledge of palliative care and end of life issues when aggressive, life-extending interventions are no longer desired. This includes the use of interdisciplinary resources.

6. Evidence-Based Practice (EBP)

Access nursing databases available through CAROL for resources to support practice. Take CHN Evidence-Based practice course or complete EBP SLP. Practice demonstrates incorporation of EBP concepts. Respond positively to evidence-based unit or organizational change.

Recognize patterns in patient presentations and intervene and / or access appropriate resources. Correctly identify and perform nursing interventions that improve patient outcomes.
