Job Summary:
 The Nurse Practitioner functions as an independent healthcare provider with an advanced degree of knowledge, skill and expertise in history taking, physical exam, treatment and clinical care management of members. The Nurse Practitioner serves patients with chronic conditions and provides exceptional care to members within Company's Care Centers, and other care environments, specializing in skilled care, long term care, and short term care rehabilitation. In this full-time position, the NP will work in conjunction with Physicians to provide care and care management to this group of patients. The Nurse Practitioner will primarily interact with members and providers at a variety of Skilled Nursing Facilities in the area.
Position Description:
 The Nurse Practitioner assumes responsibility for developing therapeutic relationships with patients, and formulates a care plan and prognosis, based on the patient's condition. They perform physical examinations and preventive health measures within prescribed guidelines in order to assist physician in diagnosing medical problems and prescribing necessary treatment and services for quality patient care. Orders, interprets, and evaluates diagnostic and therapeutic procedures to identify and assess patient's clinical problems and health care needs, explaining the necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient. Takes patient histories, conducts physical examinations, records physical findings, and prepares patient workups and summaries noting pertinent positive and negative findings. The Nurse Practitioner discusses each case with the Physician and other health professionals to prepare a comprehensive patient care plan. Submits the health care plan and goals of individual patients for periodic review and evaluation by the Physician, consulting with supervising Physician when patient's progress does not meet anticipated or predetermined criteria. Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. Refers patients to the Physician for consultation or specialized health resources for treatment. Â
Qualifications:
 Masters Degree in Nursing Required. Requires a valid, unrestricted state NP certification and RN license. Furnishing and DEA licensure in good standing with the state. The candidate must be certified to work with the adult population age group. Previous experience in a managed care organization or with geriatric patients is preferred. Candidate should have excellent people skills, with superior written and oral communication skills, and the ability to work both as a team member and independently. Intermediate computer literacy, including electronic medical record documentation, is required.  Â
