Position Summary:
POSITION SUMMARY:The Advanced RN has mastered technical and organizational nursing skills and has an in-depth knowledge of nursing practice. The Advanced RN relies on previous experiences for focused analysis of problems and solutions, and solutions are recognized as a whole. Accommodates unplanned events and responds with speed, efficiency, flexibility and confidence. Patients and families are viewed holistically, and an integrated collaborative approach to care is used. Standards of practice serve as guidelines for care and are adapted to best achieve desired outcomes.
POSITION DUTIES:
Nursing Process
1.1 Assessment - An RN uses a systematic, dynamic way to collect and analyze data about a client, the first step in delivering nursing care. Assessment includes not only physiological data, but also psychological, sociocultural, spiritual, economic, and life-style factors as well.

1.2 Problem Identification - Problem identification is the nurse’s clinical judgment about the client’s response to actual or potential health conditions or needs.
1.3 Planning and Outcomes - Based on the assessment and problem identification, the nurse sets measurable and achievable short- and long-range goals in collaboration with the patient and family. Assessment data, problem identification, and goals are written in the patient’s care plan so that nurses as well as other health professionals caring for the patient have access to it.
o Establishing daily goals with the patient and family
o Formulating a plan of care
o Transition or Discharge planning
1.4 Implementation - Nursing care is implemented according to the care plan, so continuity of care for the patient during hospitalization and in preparation for discharge is assured. Care is documented in the patient’s record.

o Documentation in healthcare record
1.5 Evaluation - Both the patient’s status and the effectiveness of the nursing care must be continuously evaluated, and the care plan modified as needed.
1.6 Care Coordination - Actions initiated by nurses with patients, families, and/or members of their healthcare team to manage and correct the sequence, timing, and/or effectiveness of patient care from admission to discharge.

5. Ethics
Identify role of CHN Ethics Committee and demonstrate understanding on how to obtain an ethics consultation. Identify that the “Ethical and Religious Directives for Catholic Healthcare Services†serves as a guideline for addressing ethical issues at CHN. Practice is consistent with the American Nurses Association “Code of Ethics for Nursesâ€. Recognize and practices within code of ethics for applicable specialty organization. Apply knowledge of palliative care and end of life issues when aggressive, life-extending interventions are no longer desired. This includes the use of interdisciplinary resources. Identify patient and/or family ethical issues and access appropriate resources including social services, spiritual care, etc.
6. Evidence-Based Practice (EBP)
Access nursing databases available through CAROL for resources to support practice.
Take CHN Evidence-Based practice course or complete EBP SLP. Demonstrate clinical leadership through organized knowledge sharing and knowledge transfer of EBP to nursing practice. Be proactive in supporting identified unit and organizational efforts to achieve optimal patient outcomes based on identified initiatives (i.e. sepsis bundle, VAP, pressure ulcers, medication reconciliation, and safety initiatives, smoking cessation).
7. Clinical Excellence
Quickly identify key components of patient presentations and initiate and / or facilitate appropriate interventions. Confidently integrate advanced technologies with clinical practice to enhance patient outcomes. Communicate effectively with peers, physicians, and hospital departments and other hospital resources to effectively manage patient care. Effectively prioritize and manage patient assignment and assume proactive role in crisis and while under stress.

Education
Required: Graduation from an accredited School of Nursing
