Skills/ RequirementsJob Description: The Field Nurse Case Manager is an experienced professional who administers skilled nursing care to the patients on an intermittent basis in the home, nursing home or other settings. He/She independently assesses, plans, and delivers this care in accordance with the physicians orders. Current and unencumbered license to practice as a Registered Nurse specific to the state of Arizona for the RN. Minimum of one (1) year of nursing experience required. Expos! ure/ experience in home health preferred. Infusion, Enteral, T! racheostomy and wound vac experience is also preferable. Current CPR certification. Able to assess patient status and identify requirements relative to age specific needs. Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars; ability to self-manage online training programs. On-Call responsibilities vary to approximately 6 days per month, staggered. Supervision: Supervises a small group, LPNs, CNAs Working Conditions and Physical Demands: Physical effort involving above-average standing, sitting, walking, detail work and driving.
