Post by Jobing - Based at our Glendale Adult Day Health Services Center, Northern & 59th Ave. Hours are fullâ"time, 35+, Mo! nday â" Friday, during the day â" NO EVENING/NIGHT HOURS AVAILABLE!STATEMENT OF WORK: Reports to Center Nurse, Site Supervisor/Center Director. Responsible for developing and implementing nursing care programs to ensure the optimal level of independence and functioning of each participant. Programs will include bowel and bladder care, restorative feeding, ambulation, range of motion, activities of daily living, transfers, and showering.
The primary function is to perform designated routine tasks related to the operations of activities of daily living. Contributes to care plan conferences and maintains confidentiality. Obtains blood pressure, weights, and reports observations about participantsâ health status to Center Nurse, Site Supervisor or Center Director. Performs any other job related tasks as deemed necessary and/or assigned by the supervisor.ESSENTIAL DUTIES AND RESPONSIBILITIES: * Assists with the health maintenance, care, and activities of daily livin! g of participants according to the care plan.* Performs routin! e procedures according to plan of care and under supervision of Center Nurse.* Develops and implements with participants, as needed, programs in bowel and bladder care, restorative feeding, ambulation, range of motion, personal living skills, transfers, and showering.* Observes and reports to Center Nurse, Site Supervisor or Center Director conditions, reactions, and responses of participants as related to assigned duties.* Records pertinent information in participantâs chart; may participate in care plan; may write progress notes.* Assembles and disassembles equipment and accessories; maintains a safe environment at all times.* Maintains confidentiality at all times.
Ensures participant privacy and dignity in all manner of care.* Assists nursing team in the maintenance of licensure and accreditation standards, including participation in the outcome measurement program.* Assists in keeping bathroom/shower room neat and clean. May be required to perform heavier cl! eaning/mopping.* Other duties as needed.COMMENTS: * Expected to respond to emergency situations involving the safety of participants, other employees, and the Center. This includes the ability to assist with a possible evacuation of participants. * Has access to individual client files and personal health information.WORKING CONDITIONS AND PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
- Works in a clear, wellâ"lit, air conditioned or heated office. * Spends a significant amount of the day standing, stooping, or bending. * Must be able to lift up to 25 lbs. * Required to assist participants with transfers.
- Required to assist participants of both genders in the use of the bathroom and shower room. * May be requested to assist participants in entering and exiting the van. * Exposure to potentially infectious bodily flui! ds such as blood or body fluids (high). * Exposure to latex, plastic, a! nd/or materials which are used in personal protective equipment.
- Demonstrates and is proficient at safety practices on the job. KNOWLEDGE, SKILLS AND ABILITIES: Minimum Required: * Must be alert to the needs of the participants.* Must have the ability to work under conditions of frequent interruptions.* Must have the ability to work with challenging behaviors expressed by participants, i.e. verbal and physical aggressions, wandering, etc.* Timely and accurate documentation.* Must be CPR/First Aid Certified.* Must have, or be able to obtain, a Level One Fingerprint Clearance Card through the Department of Public Safety background check/fingerprint clearance process.* Must have the ability to obtain Food Handlers Card. May be requested to assist in the kitchen; may be asked to assist in the serving of meal and snacks according to CACFP guidelines.* Must have the ability to assist with transfer of participants to and from chair or wheelchair.* Must not have any restri! ctions for physical work for which reasonable accommodation cannot be made.WORK EXPERIENCE AND EDUCATION: Minimum Required: * Must be 21 years of age or older.* High School Diploma or equivalent.* Must be a current Certified Nursing Assistant (CNA) in the State of Arizona, and maintain certification.
