The Unit Clerk II under the supervision of a licensed nurse performs the dual role of Unit Clerk and Patient Care Techn! ician. As a receptionist the Unit Clerk performs general clerical and other patient care duties to support the concept of Patient Centered Care and to promote an organized, efficient and smooth functioning unit. As a Patient Care Technician, this employee will assists in the delivery of care to patients in a variety of settings, within the scope of their defined skills, while maintaining accountability for own practice. Collaborates with other members of the health care team in patient care delivery, seeks assistance and support when necessary.
Education

Required:
Graduate of a Nursing Assistance program

Preferred:
Associate Degree or Diploma School Program

Certificate/Licensure/Registration
Required: BLS Provider
Arizona Certified Nursing Assistant Licensure

Maintains required Certifications/Licensures/Registrations (document expiration dates).
Completes annual mandatory education, age related! and competency assessment modules in specified time frame.
Unit Clerk Duties:
Responsible for maintaining patient paper chart organization including all required forms
Prepare charts for new patients; and organize charts for transfer and/or discharged patients following the Health Information Management policy/protocol; label chart and chart forms with correct patient/physician information
Enters physician orders into electronic record; Fax hand written med orders to pharmacy

Notifies nurse of new patient orders and/or all "stat" and now" orders.

Notifies departments of special orders.

Follows the proper sequence and procedure for ordering lab, radiology and other diagnostic/follow-up test.
Identifies policy violations and refers to Charge Nurse/Primary Nurse (e.g. orders with unapproved abbreviations, orders to resume all/continue, etc.)

Maintains unit based logs for patient admissions, transfers and discharge

Enters observation and procedure patient charge! s in clinical information system in an accurate and consistent manner.
Acts as a resource person regarding equipment, including, but not limited to, computers, printers, fax, phone, and paging systems.
Answers unit telephone, transferring calls, determining when staff members should be paged and taking and forwarding messages.
Orders and maintains appropriate inventory of unit clerical/clinical supplies; orders equipment as directed
Notify housekeeping of rooms to be cleaned promptly after patient discharge.
Monitor refrigerator temperature daily as required.
Enters electronic requisitions to appropriate department for equipment maintenance and repair
Navigate Employee Portal inclusive of Policy/Procedure
Maintain accurate nurse patient assignments and patient room assignments in the nurse call system and any other assignment communication boards.
Assists nursing personnel in answering patient call lights and reporting patient needs to ap! propriate assigned staff in a timely manner.
Provides customer serv! ice support to patients by greeting new admissions orienting them to their room, initiating information on patient dry erase board, providing a review of information booklet and introducing "Ice Breaker" card
Maintains nursing station in neat and orderly manner.
Orients and mentors new staff members
Completes special projects for unit director, supervisors and charge nurses
Serves in the capacity as an information agent for patients, visitors and colleagues for routing them to the most appropriate resource in order to avoid poor customer service experiences, within the guidelines of HIPAA.

Patient Care Technician duties:
Communicates observations and changes in patient(s) condition to the Licensed Practitioner (LP).
Delivers care to patient(s) within the technician scope of practice and as directed by the LP.
Demonstrates clinical skills in performance of procedures, treatments, and patient care.
Prepares supplies and equipmen! t for procedures and treatments using appropriate techniques.
Demonstrates knowledge in the use and care of various medical/testing equipment.
Manages supplies and linens at appropriate inventory levels.
Demonstrates accurate documentation of care provided within the technician scope of practice.
Communicates patient/family learning needs and referral needs to LP.
Participates in discharge process and patient education under the direction of the LP.
Effectively communicates patient information/incomplete assignments to ensure continuity of care.
Performs clerical duties (answers telephone, filing, photocopying, etc.) as needed.
Assists in data collection.
Other duties as assigned.
